Geriatric care management encompasses the process of planning and co-ordinating the health care of the elderly, who have physical or mental ailments to ensure long term well being, thereby enhancing their overall quality of life. In other words, it entails working with senior or elderly citizens to ensure that they get the required support to help them lead a purposeful life. Most of their impairments are age related. In this segment of health care, we provide home based health care services such as assisted living, adult, long term, residential, hospice, end of life and comfort care. Our health care plan is tailored for specific circumstances, based on the comprehensive assessment of the patient’s health state. This plan is continuously monitored and refined as required.

A thorough geriatric care assessment can take anywhere from 2 to 4 hours, which may be broken down into two or more assessment visits with the patient or their immediate family members. The initial or the primary assessment is quite comprehensive, which consists of collating the data of the patient such as health, social and financial/legal history. From this point onwards, a medication profile assessment is compiled and included, apart from compiling the assessment of activities of daily living (ADLs) and instrument activities of daily living (IADL). Apart from that, there could be other assessments that include home safety, depression, nutritional, pain and gait (ability to walk).
